Personal Information Collection Statement
The personal data collected in this Application Form (“the data”) will be used by the Hong Kong Securities Association Limited (“HKSA”) for the purposes of processing your application (“the Application”) to enroll as a member of HKSA, facilitating HKSA in its operation, development and formulation of policies, facilitating interaction between members and for providing news and event information to members.
In making the Application, it is obligatory for you to supply HKSA with the data requested in this Application Form except as otherwise indicated. The consequence for you if you fail to supply such data is that HKSA will not be able to process the Application.
The data may be provided to such persons within HKSA whose proper business it is to have access to and assist in the processing of the Application and related matters. The data may also be provided to other persons outside HKSA, who may assist HKSA in attaining the purposes above mentioned.
Any data that is provided to anyone outside of HKSA will be restricted to what is necessary and not excessive to achieve any intended purpose.
You have the right to request access to and correction of the data. Any such request should be made using the Data Access Request Form (OPS003) prescribed by the Commissioner for Personal Data and addressed to Hong Kong Securities Association Limited, Flat C, 6th Floor, Shing Lee Commercial Building, 6-12 Wing Kut Street, Central, Hong Kong.
The Privacy Policy Statement of HKSA is available on its website at www.hksa.com.hk.
If there is any inconsistency or conflict between the English and Chinese versions of this Statement, the English version shall prevail.

香港證券業協會有限公司（「HKSA」）會將報名表格所收集的個人資料（「該資料」）用於處理閣下成為HKSA會員的申請（「該申請」）、促進HKSA的運作、發展及政策制訂、促進會員之間的互動以及向會員提供新聞和活動信息。
在提交該申請時，除另有說明外，您必須向HKSA提供本報名表格中所要求的資料。如果您未能提供該等資料， HKSA將無法處理該申請。
該資料有可能被提供予HKSA內的人員，該等人員的正當事務為使用及協助處理該申請和相關事宜。該資料亦有可能被提供予其他HKSA以外協助HKSA達到上述目的的人士。

Personal Information Collection Statement
The personal data collected in this Enrolment Form (“the data”) will be used by the Hong Kong Securities Association Limited (“HKSA”) for the purposes of processing your application (“the Application”) to enroll in the Continuous Professional Training (“CPT”) courses, making logistical arrangements, compiling statistics and keeping records of your attendance, facilitating the applications for CPT accreditation of such courses and other related matters.
In making the Application, it is obligatory for you to supply HKSA with the data requested in this Enrolment Form except as otherwise indicated. The consequence for you if you fail to supply such data is that HKSA will not be able to process the Application.
The data may be provided to such persons within HKSA whose proper business it is to have access to and assist in the processing of the Application and related matters. The data may also be provided to the CPT course facilitators / workshop leaders / presenters and other persons who may assist HKSA in attaining the purposes above mentioned.
Any data that is provided to anyone outside of HKSA will be restricted to what is necessary and not excessive to achieve any intended purpose.
